Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel please print and complete the travel questionnaire below and bring this to the surgery.  The practice nurse will assess your form and then arrange for the necessary vaccinations to be put on prescription  for you to collect. The practice nurse will contact you by telephone and arrange for you to have an appointment to receive the appropriate vaccinations, you must to ensure that you have collected the prescription and received the vaccinations from the pharmacy for the appointment with the nurse.

It is important to complete this questionnaire and hand it to the surgery as early as possible - at least 8 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
